National Council

The National Council for Community Behavioral Healthcare

The National Council’s mission is to champion opportunities that will advance their member’s ability to deliver proactive and holistic healthcare services. Their membership consists primarily of healthcare organizations offering critical mental health and addiction treatment services to nearly 6 million adults, children, and families in communities across the country.

Success Story

Afia will work with the National Council for Community Behavioral Health to provide technical support in developing an IT Strategy for the Integrated Health division of the National Council, and to provide technical support for the Phase IV Primary Care-Mental Health Collaborative Care Project.

Thresholds

Thresholds

Thresholds is a fifty one year old organization located in the greater Chicagoland area that provides a variety of mental health services to over 7,000 people each year. Thresholds has a team of over 500 clinical staff that provides over a million service hours of care annually. The organization’s mission is to “assist and inspire people with SMI to reclaim their lives by providing the supports, skills, and the respectful encouragement that they need to achieve hopeful and successful futures.”

Success Story

Afia is working with Thresholds to evaluate the systems that are currently supporting the clinical, administrative, and financial functions of the organization. During this process, Afia will work to create a strategic plan to help Thresholds leverage its technology resources to be able to accomplish more clinical and business goals in the future.

Harbor

Harbor

Harbor is a behavioral healthcare provider who has been providing service in the Toledo, Ohio area since 1914. Today Harbor provides an array of services (mental health, vocational, family medicine, developmental pediatrics, day habitation, and employee assistance and wellness) to address the integrated needs of those they serve. Harbor’s 350+ multidisciplinary staff operate out of 10 locations in northwest Ohio.

Success Story

Afia began working with Harbor in December of 2009 to create an IT strategic plan to align Harbor’s technology investments to its future business initiatives. After completing the plan, Harbor continued their relationship with Afia to develop the requirements for an electronic health record system that would better support their clinical and administrative functions. During this time together, Harbor and Afia have successfully partnered to strengthen both the hardware and software infrastructure that will support all of Harbor’s distinct business areas.

Valley Mental Health

Valley Mental Health

Valley Mental Health is a behavioral healthcare provider for Salt Lake, Summit, and Tooele County residents in the state of Utah. Providing care for more than 18,000 people each year, they specialize in mental health, substance abuse and prevention services for adults, children, youth and seniors. Their mission is to be a leading community partner providing and promoting accessible, quality behavioral and physical healthcare.

Success Story

To support a newly appointed IT Director, Afia provided Valley with an assessment of their internal IT Services. The assessment provided Valley with insights into their IT operations, ideas for how to meet current and upcoming business challenges, and helped build trust and excitement between the IT department and Valley staff. Since the initial assessment, Afia has continued to collaborate with Valley’s IT Director and the Director of QI on various approaches for managing Valley’s IT Portfolio and IT Governance in a realistic and engaging way, providing transparency to everyone from Executives to Front-Line staff.

Lubbock

Lubbock MHMR

Established in 1967, Lubbock Regional MHMR Center has grown from a small service provider to an organization that develops, manages, and provides oversight for a system of many service providers, each one offering a valuable service to our consumers. It is their goal to offer people with mental illness, mental retardation, chemical dependency and HIV the opportunity to successfully achieve the outcomes they desire.

Success Story

Afia has worked with Lubbock MHMR to develop a strategic plan for evaluating and implementing new technology solutions that will better support their organization’s financial, clinical, and administrative workflow. Afia provided Lubbock with a list of strategic recommendations, and also provided them with a roadmap for organizing their IT projects, and helping to align business initiatives to technology implementations.

North Central

North Central Behavioral Health Services

North Central Behavioral Health Systems (NCBHS) is a community behavioral health organization that works within a seven county service area in Illinois. NCBHS works with over 3,000 clients and is a national leader in creating innovative clinical delivery models, including telemedicine, to more effectively serve the needs of their entire client population.

Success Story

Afia worked with NCBHS to perform a complete assessment of their current hardware and software infrastructure to allow the organization to properly support the expansion of its clinical business goals. Afia is currently working with NCBHS to prioritize and implement the recommendations that came out of this business analysis.

Functional Assessment Systems, LLC

Functional Assessment Systems, LLC is an organization started by Dr. Kay Hodges that offers assessments that can be used across various child-serving agencies as objective measures of day-to-day functioning.

Success Story

Afia has co-developed a technical framework and a systematic process for RFP evaluation and vendor certification for the development of a web application framework. Afia is also providing an IT Strategy for a variety of system deployments.

Pathways

Pathways Community Mental Health Authority

Pathways is a Community Mental Health facility that serves approximately 3,000 people in Alger, Delta, Luce and Marquette counties in the Upper Peninsula of Michigan. Pathways provides services for individuals with severe mental illnesses, severe emotional disturbances, or developmental disabilities.

Success Story

Afia has been working with Pathways since 2008 to provide training and direction on how to implement their electronic health record system, ELMER. Afia also provided a business intelligence plan and training plan to roll out a region wide reporting system.

Livingston

Community Mental Health Services of Livingston County

Community Mental Health Services of Livingston County is a mental health authority located in Livingston County, Michigan, between Detroit and Lansing.  They provide emergency services, health and medication services, client services management, individual and group therapy, assertive community treatment, older adult services, respite, substance abuse services, community supported living, wraparound and psychosocial rehabilitation.

Success Story

Afia has been successfully working with Livingston county staff to integrate accounts payable transactions from their Electronic Health Record, Encompass, to their accounting system, Munis. Afia also provides guidance and support in work flow analysis to streamline financial transactions and reporting.

Washtenaw Community Health Organization

The Washtenaw Community Health Organization is a collaborative effort between Washtenaw County and the University of Michigan Health System.  The purpose of this partnership is to provide services to persons with mental illness, developmental disabilities and substance abuse problems, while also trying to meet the primary health care needs of these populations.

Success Story

Afia successfully worked with the WCHO on projects such as refining their electronic health record, creating a personal health record, implementing a new integrated finance system, and updating their IT strategy to align with their business plan. See more »

Saginaw

Saginaw County Community Mental Health Authority

Saginaw County Community Mental Health Authority (SCCMHA) is a government organization that provides for the publicly funded mental health needs of all eligible persons in Saginaw County, Michigan. SCCMHA serves persons with serious mental illness or emotional disorders, developmental disabilities, and substance abuse disorders.

Success Story

Afia has been successfully working with Saginaw county staff to design, test, and integrate modules into the Sentri EHR. Afia has also provided Saginaw a roadmap for technology implementation that will help their organization become more mobile with their clinical teams and is working with their CIO to develop an IT strategy for the organization.

Lapeer County Community Mental Health Services

The Lapeer County Community Mental Health Services provides a wide range of professional mental health and related services for all residents of Lapeer County, Michigan, as well as for institutions and agencies serving our communities.  Lapeer serves adults, children, and families and also offers emergency and short-term counseling.

Success Story

Afia is working with Lapeer County Community Mental Health Services to develop a data warehouse that will combine key primary care and inpatient data with Mental Health data from Lapeer CMH.  Advanced coordination of care will result in better-rounded treatment of the consumer, improved satisfaction, and enhanced outcomes, both mental and physical. Once the data warehouse is established, the data will be analyzed to find certain usage patterns, to identify high utilizers, and to gain insights into treatment of chronic illnesses.  In subsequent years of the project, data will be integrated into a provider portal, a personal health record for consumers, or directly within staff electronic medical records.

Washtenaw County Public Health

The Washtenaw Community Health Organization is a collaborative effort between Washtenaw County and the University of Michigan Health System.  The purpose of this partnership is to provide services to persons with mental illness, developmental disabilities and substance abuse problems along with meeting the primary health care needs of these populations in a new way.

Washtenaw County Community Support and Treatment Services

The Washtenaw County Community Support and Treatment Services (CSTS) department provides mental health services to adults with a severe and persistent mental illness, children with a severe emotional disturbance, and individuals with a developmental disability, residing in Washtenaw County, Michigan.

Success Story

Afia has successfully worked with CSTS staff and supervisors to do a full technology assessment for their clinical teams and develop a strategy for new technology implementation.  Afia then coordinated a 3 month project to roll out over 90 new laptop computers to staff and then train staff how to use their equipment effectively in the field.
